Overview: TheCase Manager (HUD)engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition from hospital to community.

Services: HUD Case management provides post-discharge follow-up to support the consumer with community integration. The is done through direct in-home contact and linkage to community resources. The consumer is linked to resources that support the ability to remain stable within the community.

To qualify for the Case Manager position you need to have the below minimum requirements and experience

  • Minimum requirement is Bachelors degree in psychology, social work or a related field, plus 2 years of relevant experience.
  • Preferred requirement is a Master's degree in psychology, counseling, social work or a related field with clinical training.
  • Must have and maintain a valid driver's license with an acceptable driving record.

To be a successful Case Manager you must be able to do the following essential duties and responsibilities always:

Communication Requirements

  • Communicates programmatic or job-related concerns to supervisor and makes suggestions for improvement in an appropriate and timely manner.
  • Communicates any performance concerns or training needs to the supervisor in an appropriate and timely manner.
  • Conducts self in a professional manner that positively reflects the agency culture.
  • Demonstrates competent written and verbal communication with referral sources and external customers.
  • Is attentive and responsive to customer needs in a helpful, supportive and timely manner.
  • Works collaboratively with coworkers and agency staff to effectively communicate and problem solve

Supervision Requirements

  • Meets with the supervisor in accordance with program and licensing requirements.
  • Utilizes supervision effectively to address client related issues.
  • Utilizes supervision effectively to address performance issues and concerns.
  • Utilizes supervision effectively to explore professional development and training needs.

Program Quality Requirements

  • Completes a Housing Assessment for all new referrals and current residents to determine needs and choices.
  • Establish eligibility for potential tenants for HUD vouchers.
  • Assists consumers in gathering, completing and submitting application forms and other documents required to secure rental assistance and housing.
  • Assists consumers to understand Section 8 application process and documentation requirements.
  • Create and manage tenant files for all HUD tenants.
  • Assist housing applicants with housing search, including going with them to look at apartments, meeting landlords and negotiating leases.
  • Assists consumers to understand their fair housing rights.
  • Assists consumers to understand lease and tenant responsibilities and landlord responsibilities.
  • Assists housing applicants to obtain needed security deposits and community resources for household furnishings
  • Determine amount of Security Deposit for each tenant and inform tenant, and staff.
  • Perform Move In and Move Out Inspections for all HUD Voucher Residences.
  • Educates and trains tenants on household maintenance and management skills.
  • Establishes and maintains working relationships with landlords, Section 8 offices, realtors, churches, etc. in an effort to procure additional housing options
  • Ensures that all certification appointments and inspections are carried out as required.
  • Recertify tenants and renew vouchers as contract comes up for renewal.
  • Attends team meetings specific to consumer/resident care.
  • Maintains strong working relationships with housing agencies, community resources and community behavioral providers to assure access to needed services.
  • Meets regularly with Supportive Housing Manager for supervision and communication.
  • Provides essential transportation for consumers to maintain their health and wellness.
  • Attends internal and external meetings and education seminars relevant to the operation and management of the Supportive Housing Program, HUD Requirements, etc.
  • Provides on-call support and back-up as requested.
  • Assists residents with budgeting and money management.
  • Assists residents with access to community resources as needed and advocates with and on behalf of residents for needed and desired community services.
  • Submits required reports in a timely fashion.
  • Completes Progress Notes and Treatment Plans within timeframes.

  • Continually assesses clients' needs.
  • Provides appropriate intervention strategies and adjusts services accordingly.

  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
  • Must regularly be able to see, speak and hear. Frequently required to stand, sit, walk, bend, use hands and arms and must occasionally lift up to 20 pounds.
  • Must be sensitive, flexible and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation or any other special needs as reflected in the ability to communicate with staff and/or clients.
